Here are some simple exercises that will help you achieve a smooth transition from your chest voice, mid and head voice.
Try using different sounds such as:
La, pa, ma na
Mee, see lee, tree
Tie, rye, sigh, why
Row, low, bow, doe
If you try and practise these exercises most day's you will certainly improve your vocal chord and diaphragm muscles, as well as improving your chest and head voice.
